      Direct Deposit Bank Information

      The workweek runs from Sunday through Saturday. Pay for that workweek is issued by direct deposit on the Friday of the following week. Example: Hours worked from Sunday, August 2 through Saturday, August 8 will be paid on Friday, August 14.

      You may hide your account balance and any unrelated personal information. Please ensure that the routing number and account number remain visible.

        Moscoso Express, Inc. is required to verify certain employment and safety-performance information before qualifying a driver for employment.

        By completing this section, I authorize Moscoso Express, Inc. and its authorized representatives to contact the employers listed in my application to verify information related to my:

        • - Dates of employment.
        • - Positions held and commercial driving experience.
        • - Reason for leaving, where provided. Commercial motor vehicle accident history.
        • - Safety-performance history permitted or required under applicable law.
        I authorize my previous employers to provide the applicable information to Moscoso Express, Inc.

        I understand that I have the right to review safety-performance information provided by previous DOT-regulated employers, request correction of inaccurate information, and submit a rebuttal if the information cannot be corrected.
